Triple Artrodesis: resultados funcionales en pacientes con deformidad del retropié en centros asistenciales de alta complejidad, Cali-Colombia.

Abstract




Introduction: Triple arthrodesis historically is a salvage surgery for stiff painful arthritic deformities of the foot of any ethology, whether congenital or acquired in which the Subtalar, Talonavicular and Calcaneocuboid joints merge. Our objective was to describe the functional results of triple arthrodesis in patients with hindfoot deformities of congenital or acquired etiologies.
Methodology: Cross-sectional, descriptive study, type series of cases of patients undergoing arthrodesis due to hindfoot deformities for any etiology, between January 2006 and December 2015 at the Institute of Osteoarticular Diseases in high complexity care centers in Cali Colombia.
Results: 19 feet were assessed in 18 patients with an average follow-up of 57.5 months. The preoperative AOFAS improved from 23 to 74 postoperatively (p <0.05). In relation to the etiology, the reduction in the median among patients with congenital deformities was lower with a post-surgical median of 62 points compared to 76 points in patients with acquired deformities. The VAS improved in all patients, with a median of 10 in the presurgical period decreasing to 2 points after the intervention (p <0.05). In the group of congenital etiology the decrease in pain was lower compared to patients of
acquired etiology. The average SF 12 was 47.4 in the physical component and 58.7 in the mental component being higher in patients of acquired etiology. We had complications in 7 patients 38.8%, with superficial infection being the greatest complication 15.7%. Conclusions: Triple arthrodesis is a surgery indicated in the final stages of painful osteoarthritis in the pathologies of the hindfoot; This surgery improves the symptomatology and functionality in patients, whether they are of congenital or acquired etiology, without obtaining complete improvement or excellent functional results.
