Abstract
In the search to correct deformities and acquired or developmental vertebral pathologies, various surgical techniques have been proposed, this has increased the instrumentation systems and with it the associated complications, such as degeneration of the adjacent segment, related to mechanical overload and / or the iatrogenic lesion caused by the instrumentation, accelerating degeneration at the disc level and the interapophysial joints. Below we present a clinical case of a patient with posterior lumbar instrumentation, who developed Adjacent Segment Disease, and an additional procedure to less invasive arthrodesis was proposed, such as the lateral end intersomatic approach (XLIF), this is one of the safe techniques , which provides adequate structural support between the vertebral plates and can also correct deformities in the coronal and sagittal plane, thus achieving an indirect decompression of the spinal canal while respecting the posterior elements.
